Rehabilitation

Bend Transitional Care of Cascadia focuses on patient-centered rehabilitation. With our in-house rehabilitation gym, we utilize a comprehensive approach to physical, occupational, and speech therapies. By doing so, our team can work together with the patient to achieve the highest results possible. In addition to an exceptional in-house rehabilitation team, Bend Transitional Care of Cascadia is partnered with an outstanding team of physicians to ensure the most desired outcomes for each patient are achieved. Bend Transitional Care of Cascadia is pleased to offer rehabilitation therapy seven days a week by highly qualified and licensed personnel.

Post-Op Care

Bend Transitional Care has an established collaborative partnership with Bend Surgery Center to enhance the continuum of care for surgical patients. This partnership focuses on providing comprehensive post-operative observation services, ensuring patients receive the necessary care and support during their recovery process.

Meet Our Medical Providers

Dr Stanley Flemming

Dr. Stanley L. K. Flemming, DO, MA
Dr. Stan Flemming has completed nearly 40 years in the practice of medicine in addition to having a distinguished career in the United States Army and Washington State politics. He is a board-certified Family Medicine physician holding sub-specialties in the areas of Adolescent and Aviation Medicine. Additionally, he practiced in the areas of post acute/long-term care, high-risk obstetrics and HIV/AIDS Medicine having received additional sub-specialty training in these areas.
Dr. Flemming’s medical and educational experiences include hospital and private practice, public health, academic, military medicine and public health policy. He is a graduate of Pierce College (AA&S), The University of Washington (BS), Pacific Lutheran University (MA), and Western University of Health Science (DO). He completed his Residency at Pacific Hospital of Long Beach with services at Harbor UCLA and County USC Medical Center. His Fellowship was at Children’s Hospital of Los Angeles and the USC School of Medicine. Additionally, he is a graduate of the United States Army War College Defense Strategic Program, the U. S. Air Force Air University, the United States Naval War College (MA) where he graduated with Distinction, the Army Special Operations Course, and the certificated Physician Executive Program at Harvard University. He holds the academic rank of Associate Clinical Professor in Family Medicine.
In the United States Army, he served at every level of command and served two combat tours of duty in the Middle East and Kosovo, retiring at the rank of Brigadier General. Following his retirement, Dr. Flemming was appointed as an Ambassador for the U. S. Army Reserves, a 2-star position by protocol.
In politics, Dr. Flemming served in various position to include the Washington State House of Representatives, Chairman of the Board for the Evergreen State College, Chairman of the Board for the Tacoma-Pierce County Health Department, President of the Rainier Telecommunications Commission, President of the Pacific Northwest University of Health Science and Medical School, and the Pierce County Executive Pro-Tem and Councilmember. He is the founder of the City of University Place and served as the Founding Mayor and current councilmember. During the Clinton Administration, he was named one of two finalists for the position of Surgeon General of the United States. He has authored numerous articles, papers, and has published his first book. In 2022, he was the co-founder of Pacific Medical Specialty Group, LLC where he currently serves as the Chairman of the Board, Chief Executive Officer and Chief Medical Officer.

Matthew J. Davey, MD, Hospitalist

Dr. Davey is a board-certified family medicine physician who completed his residency and training at Utah Healthcare Institute. He worked as a physician at a skilled nursing facility at Ruden L.L.C. and served as the medical director at Metamorphosis Outpatient Addiction Center. Dr. Davey enjoys serving as a primary care provider for hospitalized patients, coordinating care with specialist physicians, and ensuring that follow-up care plans are communicated back to their outpatient primary care providers. In his free time, he enjoys mountain biking, skiing, running, traveling, and playing with his dog, Stella.

Tim Preston, Do

Dr. Tim Preston, DO
Dr. Preston is a board-certified Family Medicine physician, with a particular interest in inpatient and acute care. He received his medical
degree from the Des Moines University College of Osteopathic Medicine and completed his residency in Family Medicine in Coeur D’Alene, Idaho. Dr. Preston previously worked in oil and construction before transitioning to medicine, a field that better suits his personality and goals. Outside of work, he enjoys playing tennis, riding dirt and mountain bikes, skiing, playing guitar, and spending time with his family and dog.

Bradley Scott, Physician Assistant-Certified (PA-C)
Bradley Scott, PA-C, is a member of Summit Health’s Hospitalist team. He completed his undergraduate degree at Florida State University and earned a master’s degree in physiology at the same university. Bradley is a Physician Assistant since 2010, and has experience in hospital medicine, orthopedic surgery, neurosurgery, and acute care surgery. He believes in treating each patient equally and is passionate about providing primary care in an acute care setting. Bradley enjoys outdoor activities such as biking, hiking, and golfing, and is also an automotive restoration enthusiast.

Nehring, Jessica, PA-C

Jessica Nehring, Physician Assistant-Certified (PA-C) Hospitalist

Jessica Nehring, PA-C, has joined Summit Health in Hospital Medicine. She holds a Bachelor of Science in Animal Science from Cornell University and a Bachelor of Science in Nursing from the University of Utah. Jessica went on to receive her Master of Science in Physician Assistant Studies from Pacific University in Hillsboro, OR. She has certifications in Advanced Cardiovascular Life Support (ACLS), Basic Life Support, and National Ski Patrol. She has special interests in hepatology and liver disease, and enjoys providing all aspects of primary care to hospitalized patients. In her spare time, she enjoys riding her horse, skiing, hiking, biking, and pretty much everything the high desert has to offer.

Shannon Fehr, RN Case Manager

Shannon Fehr started her nursing career almost two decades ago in a hospital setting. She honed her skills as a Progressive Care Nurse and learned to balance clinical expertise with empathetic patient care. After years of frontline patient care, Shannon transitioned to Summit Health as an RN Care Manager. She coordinates and enhances patient care beyond hospital walls and applies her clinical experience to healthcare management. Shannon values safety for Summit Health patients and advocates for their needs. She finds rejuvenation in skiing, biking, and rafting, which serve as a source of renewal for her nursing profession.

Kristina Callahan, Social Services Specialist

Kristina Callahan has dedicated her career to serving the aging and people with disabilities populations in Central Oregon and Eastern Oregon since 2011. Currently, she serves as a social worker at Summit Health, where she divides her time between facilitating discharge planning for Summit patients at Bend Transitional Care and providing assistance and support for Summit primary care patients. Kristina’s passion lies in advocating for vulnerable clients in the health and long-term care setting, leveraging her extensive knowledge of Central Oregon community resources and services. Her adept case management skills have assisted numerous clients and their families in addressing and resolving social, financial, and psychological challenges related to their health conditions and securing long-term placement.
